‘Good morning, respected teachers and dear friends. Today, I am here to present a speech on health and fitness. The very known two terms to us are ‘health’ and ‘fitness’. Many times, we use these words in phrases like ‘health is wealth’ and ‘fitness is the key.’ But what do these words mean? Health implies the idea of ‘being well’. A healthy and fit person can function well both physically and mentally. Proper health and fitness do not depend solely on our efforts but also on our physical environment and the quality of food intake.

Health and fitness are the most important aspects of our lives. There is nothing more crucial than health for any human being. When we are healthy and fit, we can enjoy life fully. We can talk to our loved ones, laugh, smile, travel, enjoy food, and sleep well. Conversely, when our health and fitness are not up to the mark, we struggle to do anything properly and fall behind. We cannot enjoy eating, watching sports, or movies. Our mood remains negative, and we are unable to communicate nicely with our loved ones or even sleep properly.

Our health and fitness are more valued than our wealth. Our elders have always emphasized health and fitness, treasuring it like wealth. To maintain good health and fitness at its best, we need to take care of hygiene and sanitation around us. We must eat a healthy and complete diet, including vegetables, milk, eggs, etc. Everyone should create a diet chart and ensure a sufficient intake of carbohydrates, proteins, minerals, and vitamins. Besides eating well, we must maintain cleanliness in our rooms, houses, and nearby surroundings.

Several factors are responsible for the fluctuating health and fitness levels, but the major reason is personal habits and lifestyle. The human body requires 20 grams of fat per day. The consumption of a regular pizza adds 10 grams of fat to the body; a whopper burger adds 13 grams and 100 grams of French fries adds 15 grams. Junk foods are destroying people’s health and fitness. Due to fast-paced lives, people prioritize filling their stomachs over considering the nutritional value and health effects of their food.

As we know that health and fitness play an essential role in our overall well-being, we should strive for a balance between mind, body, and soul. Here are some suggested activities to maintain proper health and fitness:
1. Playing Sports: Engaging in games and sports helps maintain an active body, which can metabolize well and keep us in good shape.
2. Yoga: Yoga has numerous health benefits, including increased flexibility, calorie burning, improved blood flow, and boosted immunity.
3. Healthy Diet: Proper food selection leads to healthy eating. Vitamins found in vegetables and fruits, proteins found in whole grains and lean meat, and low-oil foods help control cholesterol levels.
4. Avoid Harmful Habits: Avoid smoking and drinking as they affect our sleep, which is crucial for the body to remain healthy and in good shape.
5. Regular Exercise: Exercising improves general health. One should take at least thirty minutes a day to exercise or walk.

As Dalai Lama once said, “Happiness is the highest form of health.” Happiness will come only when we are healthy and fit. But today, we live in a world where we value material things more than people, work more than family, food more than health, and status more than life. We rush after money and social status, forgetting about our health. What’s the value of life if we don’t live healthily and fit? This absurd attitude has ruined our lives and will continue to do so unless we open our eyes to reality.

Health and fitness are like the two sides of a coin. One must remain fit to maintain good health. In today’s contemporary world, it has become essential to maintain our health and physique. The quality of life is not judged by how much we earn but by how much we invest in our well-being. Health deals with the physical and mental well-being of a person. A cohesive betterment of an individual involves both physical and mental health.

Fitness is an indicator of discipline and a good lifestyle. A fit person always finds themselves at an advantage in the real world. Exercise, healthy eating, and mental well-being practices like yoga and meditation are key components of a healthy lifestyle. Let’s prioritize our health and fitness, ensuring a happy and fulfilling life.

Many of us like to have oily fried foods which are tasty but they are very unhealthy for our bodies. So we should try to stay healthy for most of the times so that we can afford to eat our favourite food sometimes, without harming our body.

Similarly, we should keep our thoughts positive and get a good amount of sleep, and fulfill all the requirements for staying in peace so that we can handle sudden shocks or stress in a manageable way.

Lack of proper exercise and diet also result in fatigue, headache and improper sleep cycle which harm us not only physically but also mentally. Lighter forms of exercise such as meditation, yoga, and walking reduce stress and give peace of mind to us after a hectic day. 

Due to the use of technology in our daily life, we have become more inactive due to the lack of any physical activity. We hardly walk to the nearest store, play on the grounds or use the staircase of our building, rather we do it all through technology without burning any of our calories. Therefore, a little physical exercise makes us tired and ill. But it's never too late and we can always start a healthy lifestyle whenever we want to. Children should opt for playing in the playground rather than spending hours on video games and phones. This will make them healthier and increase their fitness level. Although it may feel difficult to achieve at the beginning, we will be able to enjoy it and feel the difference.

Thus, I would like to conclude my speech by saying that we should take out some time out of our hectic schedule and contribute it to improving our mental as well as physical health. It is our responsibility to transform our body into a healthier version before this technology takes a toll upon our health.

Trying out meditation is also very beneficial for mental peace. Although a routine of daily exercise and healthy eating might feel difficult to continue at the beginning, one can start by taking small steps towards it. In today’s world, where educational standards are high and competition is tough, students are expected to study hard and sit in the classroom for extended periods. To ensure that students learn well and maintain good health and fitness, educational institutions need to give equal importance to physical exercises and leisure activities.

When we think of good schools and educational institutions, we often remember those with excellent facilities for sports and other activities that provide physical exercise and entertainment to the students. After extended periods of study and mental work, students require outdoor exercises to refresh their minds.

Every individual with good academic records and sound health is certain to achieve their goals in life, regardless of their chosen career. Good education and health and fitness enable a person to face challenges confidently, leading to progress and success.

A closer look reveals that individuals with good health and fitness perform better in their personal and professional lives. Physical and mental health are equally important for better performance and results. Therefore, everyone needs to prioritize good health to become a result-oriented individual.

Good health and fitness can compensate for a lack of talent in a person’s profession. Working hard on the farm to earn a living requires a lot of physical effort, but it also brings mental satisfaction and a good night’s sleep. In addition, a physically and mentally strong person can contribute more to society and receive recognition from others.

Many professions require good health and fitness as primary qualifications. The armed forces, paramilitary, police, and other forces require individuals who are physically fit and mentally alert. We have witnessed many heroic acts by individuals in these forces who have received awards for their bravery and valor under extreme conditions.

In addition, the private sector also values a well-built appearance along with professional skills. Health is of great importance everywhere, and it is an asset throughout one’s life. Positive thoughts from a healthy mind bring a sparkle to a person’s face, while negative thoughts and illness can lead to a cycle of reactions, which can have a detrimental impact on both the mind and body.

Improving healthcare and fitness is crucial because there is undeniable value in good health. However, many rural areas lack healthcare facilities and suffer from a shortage of doctors and medication. Consequently, people with multiple illnesses or injuries cannot receive adequate treatment. Even if they travel to larger cities, poor individuals may be unable to afford private hospital treatment for major diseases.

Despite the importance of medical facilities, increased awareness is necessary for maintaining good health and physical fitness. Individuals who are mindful of their health and fitness can join yoga camps or gyms, serving as a source of inspiration for others. By adopting healthy lifestyles, people can become disease-free and enjoy better health and well-being.

Furthermore, freeing oneself from unhealthy and negative thoughts like self-indulgence can foster the will to improve one’s body and mind, reducing illnesses with psychological origins. As a result, individuals can experience superior mental and physical health.

1. Introduction

Regular physical activity and exercise can improve your health and reduce your risk of developing various diseases including type 2 diabetes, cardiovascular disease, and cancer. Physical activity and exercise can have numerous immediate and chronic health benefits, such as managing weight, blood cholesterol level, blood pressure, and muscles [ 1 ].

Most importantly, regular physical activity and exercise can offer a better quality of life (e.g., more energy, a better mood, feeling more relaxed, and sleeping better). The American College of Sports Medicine (ACSM) provides recommendations and guidelines for physical activity and exercise that all healthy adults aged 18–65 years should participate in moderate-intensity aerobic activity for a minimum of 30 min five days per week or vigorous-intensity aerobic activity for a minimum of 20 min three days per week. In addition, adults should maintain or increase muscular strength and endurance for a minimum of two days per week (ACSM’s Guidelines for Exercise Testing and Prescription) [ 2 ].

Taken together, both aerobic and strength activities are important for optimal physical fitness. While practical concerns such as busy schedules and poor health can make exercise more challenging, for most of us, the biggest barriers are mental. Maybe it is a lack of confidence that keeps us from taking positive steps, or it is that the motivation easily burns out, or that we are too quickly discouraged and give up. Therefore, new exercise methods that enable exercise to be sustainable for managing health are essential. Among the many new exercise trends, we would like to briefly introduce some of the exercise modalities including blood flow restriction, electromyostimulation, hypoxic training, vibration, and interval training.

2. Blood Flow Restriction

Blood flow restriction training is an effective exercise method developed by Dr. Sato Yoshiyaki of Japan in 1966 that uses a blood flow control band to increase growth hormone secretion and improve blood circulation and metabolism to prevent and improve adult diseases, shorten rehabilitation periods, and enhance cardiovascular function. The purpose of blood flow restriction training is to temporarily restrict venous blood flow to the peripheral area of the extremity through pressurization and to improve blood flow through exercise after pressurization [ 3 ]. In general, strength training has guidelines such as intensity, frequency, shape, set, and number of repetitions of exercise [ 2 ]. In traditional training, exercise with a load of more than 65% of 1 RM can be expected, but in blood flow restriction training programs, a 20 to 50% level of 1 RM is maintained, and 1 set is performed between 25 and 30 times [ 4 ]. In the case of this training, the upper and lower body should be recessed for 15 s and 30 s, respectively. Moreover, the frequency of daytime exercise is not as tiring as traditional muscle exercise, and recovery is faster, so pressure can be adjusted in six steps depending on the exercise’s characteristics or purpose [ 3 ]. Blood flow restriction training was conducted twice a day at 20% of 1RM for 2 weeks, showing an effect of increasing the quadriceps muscles’ cross-sectional area by 7.8% and the muscle fiber’s cross-sectional area by 5.9% for type 1 and 27.6% in type 2 [ 5 ]. In addition, blood restriction training can lead to aerobic to anaerobic metabolic transition in low-intensity exercise due to blood flow restrictions and congestion, which leads to the strengthening of the hypoxic environment and muscle nerve activity and activation of the endocrine system. When blood flow is restricted, oxygen supply to the muscles is not smooth, and removal of metabolites such as lactic acid is hindered. As a result, exercise units including multiple fast muscle fibers are mobilized to maintain muscle strength [ 6 ]. Muscle training is also different from conventional resistance exercise in that muscle training is performed in the order of muscle fibers and depending on strength [ 5 ]. As such, low-intensity blood flow restriction can have the same effect as high-intensity resistance exercise, and it is also effective as a rehabilitation exercise for elderly or sick people with sarcopenia [ 7 , 8 ]. However, blood flow restriction training is not always effective. The side effects during blood flow restriction training are still very rare [ 9 ]. The most common side effect is subcutaneous bleeding, which occurs temporarily for a short time and decreases with continuous training [ 6 ]. In addition, venous thrombosis, rhabdomyolysis, and cerebral anemia symptoms have been reported rarely, but no serious side effects or complications have been caused [ 9 ]. There are no cases of death from pressurized training compared to 0 to 2.5% [ 10 ] in extreme exercise among older people, so pressurized training is considered a safer and more effective training method for patients with fractures, diabetes, high blood pressure, and cerebrovascular diseases. [ 11 ].

3. Electromyostimulation

Electromyostimulation training (EMS training) is a principle in which a current stimulates the muscle nerves to cause artificial muscle contraction and is used for muscle strength [ 12 ]. In the past, it was mainly used for the improvement and treatment of muscular nervous system dysfunction, the prevention of postoperative muscular atrophy, and pain relief [ 13 ], but in 1977, Kots in Russia showed positive effects after applying EMS training to elite athletes. Studies have been conducted on various subjects, including elite athletes and the elderly, as well as healthy adults [ 14 ]. In particular, sports medicine is applied to the field for the purpose of strengthening muscle strength, rehabilitation, edema control, and recovery to improve performance through EMS training [ 15 ]. The increase in muscle strength caused by the implementation of EMS training can be explained by a mechanism similar to the increase in muscle strength by the high-strength, low-repeated muscle strength protocol performed in general resistance exercise. Seyri & Maffiuletti [ 16 ] stated that EMS training significantly improved muscle strength and anaerobic power in healthy adults and increased voluntary muscle activity. Wall et al. [ 17 ] reported an increase in muscle synthesis after EMS treatment in older adults with diabetes. However, despite these many advantages and limitations, existing EMS training is passive and limited to local areas, so full-body strength training is limited [ 18 ]. Deley & Babault [ 19 ] reported improvements in cardiopulmonary ability through EMS training [ 19 ]. To compensate for these shortcomings, Whole-body EMS (WB-EMS) training equipment has been developed to perform full-body exercises and has become a popular movement in Europe since the early 2000s, mainly in Germany [ 20 ]. WB-EMS training is a new type of training based on the latest electrical stimulation control technology that allows short-time, high-intensity exercise to be performed with less movement, and it is considered a very suitable exercise for people who are not interested in exercise or who are physically challenged [ 18 , 21 ]. WB-EMS training has been reported to increase the protein synthesis of skeletal muscle in older adults in inactive frailty precursors [ 22 ]. In particular, WB-EMS has been reported to have a positive effect on the body composition of older people with low physical activity and a high risk of sarcopenia [ 21 ]. In a study of cardiovascular variables for WB-EMS, it was reported that mean blood pressure (MAP) decreased by 4.9% and 8.1% in older women after WB-EMS training, significantly improving the cardiovascular system [ 23 ].

4. Hypoxic Training

Altitude training, also known as hypoxic training, involves exercising in, living in, or otherwise breathing oxygen-reduced air. This is performed to improve athletic performance and physical wellness. Training under a state of hypoxia can also help people acclimatize to higher altitudes. Since the 1968 Mexico Olympics (2240 m) held in Mexico City, many studies have been conducted on altitude training to improve the performance of aerobic elite athletes [ 24 ]. In order for elite athletes to achieve successful results in international competitions such as the Olympics and world championships, they require at least six years of athletic experience and at most 16 years of experience. Elite athletes try to improve cardiopulmonary endurance, an important factor in order to achieve good results in competitions. Altitude training is one of the training methods which has been referred to as “altitude”, with elevations of more than 3000 m above sea level, and is relatively flat, primarily with steep cliffs, located above 500 m above sea level from a geographical point of view [ 25 ]. Based on this, some elite track and field athletes conducted altitude training in Chamonix, France, Albuquerque in the United States, and Kunming in China [ 26 ].

A low-pressure and low-oxygen environment means an environment in which the oxygen partial pressure in the atmosphere is lower than the pressure at sea level (760 torr). Exposure to this low-pressure and low-oxygen environment results in physiological adaptation at the level of blood components that affect the oxygen-carrying capacity of the blood [ 27 ], although the degree of adaptation varies depending on the period of exposure. Physiological adaptation due to exposure to high-altitude environments is caused by mechanisms to compensate for the reduction in alveolar oxygen partial pressure (P A O 2 ), arterial oxygen saturation (%SaO 2 ), arterial oxygen content (CaO 2 ), and sympathetic oxygen difference (a-vO 2 diff) in the body due to reduced oxygen partial pressure (PO 2 ). In addition, aerobic exercise ability is reduced during exercise in such low-pressure and low-oxygen environments [ 28 ]. In endurance sports, exercise performance is greatly influenced by oxygen transport capacity from the whole body to local areas such as active muscles (Park et al., 2018). This improvement in oxygen transport capacity results in an increased efficiency of aerobic energy production, resulting in increased maximal oxygen uptake (VO 2 max), as well as improved time to fatigue during exercise and increased exercise intensity [ 29 ]. Training in natural and artificial altitude environments has been reported to improve the performance of aerobic sports by improving their oxygen transport and utilization through hematologic changes such as hemoglobin (Hb) mass increase and red blood cell (RBC) production and cardiovascular function and skeletal muscle oxygen availability. Recently, some researchers have applied a natural artificial notification environment to improve muscle function through resistance training. As suggested by Schoenfeld et al. (2013), this principle is based on the mobilization of muscle fibers with increased metabolic stress due to exercise, increased hormone release, changes in myokine production, increased production of reactive oxygen species, and cell edema to induce muscle hypertrophy and muscle function improvement [ 30 ]. In the case of studies related to metabolites, several evidences argue that resistant exercise training performed in a low-oxygen environment can lead to greater stimulation of metabolites to improve muscle hypertrophy and muscle function, but the correlation is not clear [ 31 , 32 ]. Park & Lim [ 33 ] showed an improvement in muscle strength and muscular endurance and an increase in the viscous endothelial growth factor (GH, IGF-1, VEGF) through the application of 6 weeks of resistive band exercise training by swimmers in a low-oxygen environment. However, it could not explain the correlation between muscle function and hormones. In particular, the current role of systemic endocrine responses in muscle hypertrophy and muscle function improvement is controversial among many scientists. Therefore, additional mechanical research is needed.

5. Whole Body Vibration Training

Traditional training, such as aerobic exercise or resistance exercise, is effective in improving constitution and fitness [ 34 , 35 ] but may be unsuitable for those who have difficulty performing the desired intensity and amount of exercise, especially for obese or elderly people [ 36 ]. WBVT refers to a method of stimulating a specific muscle group by vibrating the whole body. It is mainly driven by standing on a vibrating scaffold and can perform a variety of motion movements in situations where vibration is applied without external resistance [ 37 ]. These vibrational stimuli (to be precise, amplitude and frequency) mechanically stimulate intrinsic receptors such as the proximal spindle, which can lead to the response and adaptation of the muscular nervous system to enhance muscular endurance and muscle strength [ 38 ]. As a result of applying WBVT to obese people, most studies reported improved body composition, muscle strength, blood pressure, and blood variables (TG, LDL-C, insulin, leptin, adiponectin) and improved PWA, PWV, and Aix predicting cardiovascular disease, which were more effective in obese women. However, an accurate protocol has not yet been presented for exercise intensity (amplitude, frequency, exposure time), and it is reported that there will be a difference in effect. Therefore, considering the short research period of prior studies, it was suggested that a longer study would be necessary [ 36 ]. The study of applying WBVT to older adults showed a better effect on bone density (especially important femur, hip, and lumbar) than traditional training [ 39 ], but conflicting results suggest that more research is needed [ 40 ]. Most studies applying WBVT to older adults showed no better muscle strength compared to traditional muscle strength training, but showed improvements compared to controls, and improved functional strength important for preventing falls such as stride, walking speed, reaction speed, and SLR [ 37 ].

6. Interval Training

Interval training refers to an interval set according to the training intensity (or time, distance) and a training method that exercises with relatively low intensity for the number of intervals desired for recovery. The basic principle of interval training is to exercise at a high exercise intensity during a required distance or exercise time, then exercise at a low exercise intensity for a recovery period, and then exercise again at a high exercise intensity [ 41 ]. The purpose of interval training is to train players to familiarize themselves with the high performance required when competing against other players in competitions so that they can perform well during a given exercise time and pace. Interval training usually takes place a few weeks before a race [ 42 ]. In the initial stage of interval training, high-intensity exercise for a short period of time and long rest periods should be set. After becoming used to these training methods to some extent, the exercise time increases and the rest time shortens to increase the amount of exercise and interval intensity [ 43 ]. Eventually, the focus turns to exercise intensity, and if not, the focus is on exercise distance (e.g., marathon race). The large division of interval training consisting of 1. High-Intensity Interval Training (HIIT) and 2. Sprint Interval Training (SIT” supramaximal”) has a greater effect than traditional medium-intensity exercise on improving mitochondria, which are important in terms of maximum oxygen intake and aerobic function [ 44 ]. It has also been reported to be more effective in improving cardiovascular disease, metabolic disease, blood pressure, body composition, and physical strength than traditional medium-intensity exercise, and it has been suggested as a very effective exercise method for the elderly and obese people who are vulnerable to the above risk factors [ 45 , 46 ]. In most studies applying HIIT to the elderly, muscle strength, cardiopulmonary endurance, blood TG, and functional fitness were improved, which can lower the risk of sarcopenia, which should be considered significant in the elderly [ 45 ]. This effect was most effectively suggested by repeating 40 min of exercise and 6 sets twice a week for more than 12 weeks and within 60 s of training and 90 s of rest. When HIIT was applied to obese people, it was very effective in reducing waist circumference and body fat, and 10 weeks of HIIT reduced body fat without weight change [ 46 ]. In particular, when comparing the effects by applying exercise time such as traditional medium-intensity training, it seems to be suitable for those who lack time. These effects are known to be ineffective when not combined with diet [ 47 ], but when comparing moderate-intensity training and HIIT, only HIIT shows a significant decrease in visceral fat, so exercise should be combined to effectively improve abdominal obesity [ 48 ].

The Department of Movement Sciences offers Idaho Fitness courses (IFIT 106, 107, 108, and DAN 105), open to all U of I students, staff and faculty members. The mission of the IFIT Program is to enhance the holistic well-being of the university community and guide each individual in discovering and enjoying a sustainable healthy active lifestyle.

  • You will need a PVG check through Disclosure Scotland in order to go out on the placement(s) as part of this course

  • Intermediate Certificate: Sport and Fitness (minimum of 13 assessed SQA credits)
  • Positive Internal Learner Reference

All applicants will be invited for an information session

Thanks very much for inviting me here today.  

A lot changes from opposition to government. I’ve gone from an office of seven staff to a department of 3,500. People insist on calling me by my title, rather than my name. The most frustrating change, and I am sure that I won’t be the first to have said this, has been the pace – or lack of it - at which the Whitehall machine works.  

So for Ara to have published two reports in the past two weeks, both over a hundred pages long, packed with expert analysis and huge amounts of data, the first of which he wrote in just two months, is really putting the British state to shame. Especially on the first of those reports he put together in just over two months. He’s on a one-man mission to solve our country’s productivity. 

The Darzi investigation into the NHS and the final report from the IPPR’s Health and Prosperity Commission complement each other. Taken together, alongside the OBR’s Fiscal Risks and Sustainability report, they depict a perfect storm of rising pressures on the NHS, growing numbers dropping out of the workforce, and spiralling costs to the public purse, at a time when there is already a £22 billion black hole in the public finances. 

This is what we’re up against. I’ve been in the Department of Health and Social Care for two months, and day after day I’m shown graph after graph with all the lines going in the wrong direction. 

Recognising this is criticised by some for being ‘gloomy.’ It’s not actually, it’s something this country hasn’t been used to for a long time- it’s honesty.

This government has resolved to be honest about the problems facing our country, and serious about solving them. 

You saw both in Keir’s speech last week- honesty about the state of our NHS, and a serious agenda for turning it around. As the Prime Minister set out in his speech, continuing with the status quo isn’t an option. Sticking plasters won’t cut it. We’re going to have to make big changes, many of which will come up against loud opposition. So don’t be surprised that we’re making the case for change at the same time. 

If anyone was in any doubt about the scale of the challenges before the NHS, then the Darzi investigation published last week made it abundantly clear. 

I asked Ara for hard truths, warts and all. He delivered. And then some. He has given a raw and honest diagnosis: 

The promise that the NHS will be there for us when we need it has been broken for almost a decade 

100,000 infants, aged 0-2, were left waiting more than six hours to be seen in A&E last year 

Cancer is more likely to be a death sentence for NHS patients than patients in other countries 

Fifty years of progress on cardiovascular disease is going into reverse 

And patients have never been more dissatisfied with the service they receive 

The report is equally damning on the causes. 

First, a decade of underinvestment…  

…which left the NHS 15 years behind the private sector on technology… 

…with fewer diagnostic scanners per patient than almost every comparable country, including Belgium, Italy, and Greece… 

…and mental health patients treated in Victorian cells, infested with vermin, where 17 men are forced to share two showers. In 2024. 

As the report states and I quote:

“It took a scorched earth approach to health reform…. the effects of which are still being felt to this day.” 

Just imagine, if all the time, effort, and billions of pounds wasted on dissolving and reconstituting management structures had instead been invested in services for patients.  

Clearly, the NHS would not be in the mess it finds itself in today. 

Third, Coronavirus. 

The damage caused by the pandemic has been plain to see. 

But what we didn’t know until the Darzi report, is that the pandemic hit the NHS harder than any other comparable healthcare system in the world. 

Patients in this country saw far more operations and routine care cancelled than anywhere else.  

That’s why the backlog ballooned to 7.6 million today.

Fourth, the failure to reform. 

A 17% increase in staff working in hospitals, which didn’t lead to better outcomes for patients, but a huge fall in productivity.  

Staff put into the wrong place, without the tools to do the job.  

Instead of treating patients, too many are trying to find them beds, find somewhere to discharge patients, or dealing with slow, creaking, outdated IT. 

An enormous waste of talent and money, when so many other parts of the NHS were crying out for investment.

Activity-based funding was ditched in favour of block contracts. Incentives to reform and modernise ways of working have been stripped out. Translated into plain English, too many parts of the NHS are now paid for effort, not for outcomes. That has to change. And we’ll be saying more about that in the coming months. 

Some of the most shocking findings in the Darzi investigation weren’t about the sickness in our National Health Service, but the sickness in our nation. 

Children are sicker today than the generation a decade ago. Adults are living longer, but falling into ill-health earlier in life. And as this report from the IPPR puts it, we are in danger of literally becoming the sick man of Europe. 

The result is a rising tide of pressures on the NHS. 

I’ve said it before but I don’t think the message has yet got through: if we don’t act now, ever increasing demands for healthcare threaten to overwhelm and bankrupt the NHS. 

Our sick society is also holding back our economy. 

A drop in productivity due to ill-health has cost our economy £25 billion since 2018 

900,000 more people are off work than would have been on pre-pandemic trends. That’s more people than Tesco, Sainsbury’s, and Asda employ put together.  

Fail to act, and by the end of this parliament 4.3 million people could be off work sick. Millions of people left behind, the welfare bill ballooning, and growth hampered.   

To build a healthy economy, we need a healthier society.  

To achieve that, we need reform of the health service, and public health reform too. 

Reform has always had opponents, often vocal and powerful opponents. But believe me when I say the Prime Minister and I will face down that opposition, because it is in the interest of patients, the health service, and taxpayers in this country. 

It really is reform or die, and we choose reform. 

As the Prime Minister said last week, radical surgery is required. We will publish a ten-year plan early next year that will set out how we deliver three big shifts in the focus of the NHS: 

From Analogue to digital 

Hospital to community 

And sickness to prevention 

And what gives me confidence that this can be done is the innovations already taking place in our health service and our country today, pointing the way to a better future. 

At the Royal Free Hospital in London, the hospital which treated my kidney cancer, new PET scanners work four times faster than standard. Patients are seen days, not weeks after being referred, cutting delays in diagnosis that can be the difference between life and death. 

Think of the number of lives that could change, if rolled out across the country. That is why we are committed to putting an AI-enabled scanner in every hospital- freeing up valuable staff hours and getting patients diagnosed much faster. 

In the South-East of England, virtual wards have led to 9,000 fewer hospital admissions per year. Patients living with long term-conditions like heart failure are cared for from the comfort of their own home, freeing up vital hospital beds and saving more than ten million pounds.  

Our commitment to invest in more district nurses will make sure more patients across the country can get the treatment they need without having to step out the front door. 

Better for patients, and less expensive for the taxpayer. 

As the Prime Minister said last week, by marrying our country’s leading scientific minds, with the care and compassion of the NHS, the fruits of the revolution taking place today in life sciences can be secured for NHS patients. That public-private partnership will be fundamental to the reform agenda. 

Take MS patients. Right now, many are treated through a four-hour blood infusion – a long and difficult ordeal.  

Soon, thousands will be offered an injection that takes just ten minutes. Just imagine the difference this will make. The hours returned to patients to live their lives. And the thousands of hours of clinicians’ time freed up for the millions of other patients stuck on waiting lists. 

And when it comes to prevention, the healthy society we need to build won’t just come from the action we’re taking as a government, to ban junk food ads, pass the tobacco and vapes bill, or introduce health checks in workplaces and on high streets. 

Nor will it only come from the reforms we will make so the NHS can reach patients sooner, diagnose earlier, and treat them faster. 

In May, we saw the first NHS patient to be treated with a personalised cancer vaccine at University Hospitals Birmingham.   

These vaccines could prevent cancers from returning after surgery, sparing thousands of patients and their families the torture of recurrence. 

That’s why I am determined to rid my department and the NHS of any cultural sniffiness about dealing with the private sector, make the NHS the best partner in the world for the life sciences sector, and make sure NHS patients can enjoy the benefits. That is what Keir Starmer’s mission-driven government is all about. It’s about mobilising the public sector with business, the voluntary sector and civil society to achieve big lasting change for our country. 

In doing so, we won’t just be building a healthy society, but playing our part in contributing to the government’s number one mission - economic growth. 

So in conclusion - we’ve hit the ground running in the last two months, bringing an end to the junior doctors strike which had lasted 18 months, cost patients 1.5 million cancelled operations and appointments, and cost taxpayers a staggering £1.7 billion.  

And we’ve begun to fix the front door to the NHS, with an additional 1,000 GPs hired by the end of the year, beginning to reverse the decline in primary care of the past 14 years. 

It’s 76 days since the general election and 76 years since the founding of the NHS. There are no quick fixes for an institution of this size and age. Especially after the damage done by the Conservatives of the last 14 years. 

None of this is easy and it will take time.  

There will be more arguments to be won along the way, against the ideologues who don’t believe the NHS should have any relationship with the private sector, and the modern-day luddites who resist greater use of technology in the health service. 

This government will never be afraid to take on the arguments, and take the difficult decisions, to deliver the change that the British people voted for. 

If we see this through, the prize is huge. A healthy nation, where fewer lives are lost to the biggest killers, and fewer people are forced out of work through preventable disease.  

A healthy, growing, prosperous, economy with the proceeds of that wealth shared across our country. An NHS there for us when we need it once again – recovered, reformed, sustainable for the long-term. That’s what we will do.
