Case Studies That Define Mechanical Engineering Challenges

Mechanical engineering is key in creating and perfecting the machines and systems we rely on every day. To truly grasp its complex challenges, it’s useful to look at detailed case studies.

These studies not only show how engineering works but also highlight why keeping people safe, coming up with new ideas, and making ethical choices matter so much.

Let’s explore some major events that tested mechanical engineering. The Hubble Space Telescope Repair

Mechanical engineers faced tough problems when fixing the Hubble Space Telescope. To tackle these, they combined deep knowledge with creative thinking. They carefully checked the telescope’s parts to find what was broken.

Then, they came up with special tools and steps for the astronauts to fix the telescope in space, where conditions are very difficult. They used a method called finite element analysis to make sure the telescope would be strong enough after the repairs, and they made sure that new parts could be swapped in easily.

Their detailed planning and work didn’t just get Hubble working again; it also made sure the telescope could keep working for a longer time. This shows how important mechanical engineering is in solving tough problems with machinery in space.

Mars Rover Mobility Dilemmas

Building Mars rovers is a tough job for mechanical engineers. They have to make sure these rovers can move smoothly over Mars, which has a very rough surface with lots of rocks, sand, and big hills. Engineers work with advanced robot technology, new types of materials, and knowledge about other planets to solve these problems.

They test their designs over and over to see how the rover parts will handle the ground on Mars. The parts have to be really strong and not wear out quickly, even when they hit unexpected bumps.

Also, the rovers need smart computer systems that can figure out the lay of the land by themselves and change their path to avoid getting stuck or broken. This is super important because if something goes wrong with the rover, it could mess up the whole mission to Mars.

Tacoma Narrows Bridge Collapse

The Tacoma Narrows Bridge collapse is a key example for mechanical engineers of why it’s essential to consider how wind affects bridges. When the bridge fell apart in 1940, it showed that suspension bridges can swing and twist dangerously in the wind.

The bridge’s narrow shape, the solid pieces it was made of, and not enough side support were big reasons why it moved so much and then broke. This disaster made it clear that testing bridges in wind tunnels was necessary.

As a result, bridge design changed a lot. Engineers started using special devices to stop bridges from swinging too much and created stronger design rules. These changes help make sure that big bridges can stand up to wind without getting damaged.

Fukushima Daiichi Nuclear Disaster

The Fukushima Daiichi nuclear disaster is a crucial lesson in the need for strong and reliable engineering. On March 11, 2011, a huge earthquake, with a strength of 9.0, shook Japan and caused a major failure at the Fukushima Daiichi Nuclear Power Plant. This event showed how important it is for machines and structures to be able to survive natural disasters like earthquakes and tsunamis.

After the earthquake, a tsunami followed, which made things even worse. The power plant lost power, which meant the cooling systems for three of its reactors stopped working. Without cooling, these reactors overheated and their cores melted down. This was a tragic example of what can go wrong when we don’t carefully think about the risks of building in areas where natural disasters can happen.

Later on, experts looked at what went wrong and found that the power plant wasn’t built to handle a tsunami of that size, and the emergency plans weren’t good enough for such a disaster. Now, the Fukushima disaster is a key example that engineers study. It shows the link between planning ahead, considering the environment, and the serious problems that can happen if we don’t.

Deepwater Horizon Oil Spill Engineering

In April 2010, the Deepwater Horizon oil rig failed disastrously, causing a huge oil spill in the ocean. This event was significant because it showed how much we needed to improve our deep-sea drilling technology and how we respond to such disasters. Engineers had to look closely at how they build and check the safety of underwater equipment, such as blowout preventers, which are supposed to stop leaks. They also needed to make sure oil wells were designed to be very strong and could handle emergencies.

The spill made it clear that we must do better at predicting risks, watching over drilling as it happens, and creating tools that can handle the tough conditions at the bottom of the sea. After the spill, experts worked on making better plans for how to deal with such problems, which included using more advanced robots that can work underwater and creating better ways to quickly block leaking oil wells.

Engineers learned a lot from what happened with the Deepwater Horizon. They are using those lessons to try to make drilling in deep water safer, to reduce the chance of another big oil spill. This means continuing to develop new technology and safety measures that can prevent or quickly stop leaks if they do happen.

In summary, every case study shows different problems that mechanical engineers have to deal with.

For example, fixing the Hubble Space Telescope needed very accurate work, and getting the Mars Rover to move on Mars took a lot of creativity.

The fall of the Tacoma Narrows Bridge and the nuclear accident at Fukushima Daiichi teach us that strong design and having a backup plan are very important.

The oil spill at Deepwater Horizon reminds us that safety features and thinking about the environment are essential.

These examples show that mechanical engineering involves many different areas and that it’s important to keep coming up with new ideas and to watch out for possible risks.

What are Cases and Why are they Important?

Watch this video as an introduction to case studies and their value in education:

Engineering case studies are a representation of an engineering practice, event or challenge. They have a realistic context and appropriate complexity. Cases motivate learning through the realistic application of engineering principles and theories, through analysis and design.

Waterloo Cases in Design Engineering uses a modular case structure, with the first module presenting the case itself. Subsequent modules present stages in the solution, and are made available to instructors for guidance on case use.

Case teaching resources are available for instructor use. They can include a teaching note, presentations and simulations, and activities that have been used by other instructors.

Cases and Learning

Cases can be used in a variety of ways to teach content, and to involve students with real-world situations, to help them develop appropriate knowledge, skills and attitudes. In comparison with classical lecture methods, case-based approaches have been shown by educational researchers to improve different aspects of teaching and learning, reasoning and problem-solving skills, the ability to make objective judgments, cooperative and active learning the ability to identify relevant issues and recognize multiple perspectives, and awareness of ethical issues.

Cases can be used in a number of ways to improve understanding:

  • as example problems,
  • in-class projects,
  • basis of class discussions to illustrate an engineering principle or process, including design.

Learning with cases is an active process. It requires individual work to gain an initial understanding of the case, but learning is significantly enhanced through active discussion with fellow students and the instructor. Case learning provides an excellent forum for the development of teamwork and communication skills in students.

Case Studies for Engineering Ethics Across the Product Life Cycle

This activity is considered an NAE Exemplar in Engineering Ethics Education and was included in a 2016 report with other exemplary activities. This activity uses reviewed case studies and life cycle assessment tools to help students develop needed ethical decision-making skills. 

Exemplary features: Adaptability for use in secondary education; extensive collection of cases on the ethics of lifecycle impacts and sustainability

Why it’s exemplary: Real-world engineering decision making involves multiple actors and, for each, ethical considerations may arise at multiple levels—personal, professional, societal, or global. Our program of case studies and educational materials is exemplary in its interdisciplinary foundation, created collectively by engineers, policy experts, business professionals, and ethicists to provide clear examples for rising engineers to appreciate ethical issues from multiple angles. Accompanying materials are rigorously assessed in the classroom by internal and external evaluators based on national educational goals and guidelines, with versions developed to suit a variety of instructional modes. Full cases are designed for university engineering students, while streamlined versions for secondary schools spread an awareness of lifecycle issues and environmental ethics early in formal education. Widespread dissemination using various media adds to national infrastructure for ethics education in engineering and environmental fields, with the goal of emphasizing societal ethics and indirect effects.

Program description: A central goal of engineering education is to provide students with an understanding of context for their designs and decisions. A common theme currently relates to the environment and public health, specifically what constitutes a fair distribution of emissions or impacts, who or what has value, and what exactly gets counted in an engineering analysis of benefits and costs. These questions can be quite effectively discussed in the context of lifecycle engineering, a design strategy that uses a “cradle-to-grave” approach to evaluate environmental and social impacts, incorporating material, energy, and economic flows as well as social and biological effects at different stages. While the use of lifecycle engineering and lifecycle assessment (LCA) tools is widespread, the modeling structure and interpretation of results involve ethical and value judgments that must be navigated carefully by the analyst and by the receiver of the results.

LCA is increasingly important in corporate and government decision making, yet there is a dearth of materials specifically designed to integrate ethics education into life cycle–oriented coursework. Our ethics education project centers on the integration of life cycle–oriented case studies in design, engineering, management, and public policy fields. Case studies are effective pedagogical tools, and particularly useful in enabling students to develop practical understanding of the ethical challenges they will face as practicing professionals by placing them in mock decision-making roles. We have conducted a thorough review of nearly 1,000 existing case studies from engineering, business, and public policy to determine common topics and themes that relate to product life cycles and environmental and health impacts. Our case studies cover current events and engineering design decisions that involve balancing local or direct effects with larger, indirect effects on society, including (a) mismanagement of industrial waste and ecological impacts from industrial accidents, specifically the inundation of several villages in Hungary from a large-volume spill of red mud, a byproduct of aluminum production (production stage); (b) the upstream implication of material selection for consumer electronics, specifically the tradeoffs between Au-coated antennas and GaIn liquid metal reconfigurable antennas, a new technology being piloted by handset manufacturers (design stage); (c) implementation of state-level policy around compact fluorescent bulbs, balancing state targets for energy efficiency, indirect emissions as a result of reducing electricity demand, and direct potential emissions of Hg during lamp breakage, both accidental and intentional (use and disposal stages); and (d) whether federal/state agencies could and should require labelling of nanomaterials in consumer products, drawing parallels with labelling efforts for pharmaceuticals and food (use and disposal stages).

Following typical case study methods, students are presented with an engineering or design decision that they need to make, accompanied by background material that provides technical, environmental, and policy context. An accompanying teaching note guides instructors with ideas for classroom instruction, emphasizing the ethical concepts that are relevant to the case and written with proper terminology in collaboration with the Ethics Institute at Northeastern and assessed by an external evaluator. Instructional materials and video footage presenting each case, as well as shorter versions for younger audiences, are being created and will be hosted at the Ethics Institute as an additional teaching resource. The creation of the case studies involved a multidisciplinary collaboration among faculty members as well as graduate students. Undergraduate students and high school teachers are assisting in the creation of versions appropriate for secondary schools. These cases have been designed as one-week modules to be incorporated in existing courses and ethics workshops.

The educational goals of this project are to:

(1) Create engaging, practical, and effective case study and workshop materials that examine ethical dimensions of LCA practice and communication, for use in courses in engineering, management, and social science;

(2) Evaluate the effectiveness of these materials through robust educational assessment while improving student learning; and

(3) Engage other secondary school and college/university instructors through demonstration and provision of instructional guides and resources to accompany the case study and workshop materials.

The overall purpose of the project is to enable engineering students and the general public to have an understanding and meaningful discussions of indirect impacts of their activities, and how to balance direct benefits and indirect impacts. Our life cycle–oriented, case-based approach to engineering ethics education will fill gaps in case study resources by addressing fundamental ethical principles and macro-ethical issues on sustainability topics, developing novel, robustly assessed educational materials where few currently exist.

Assessment information: Our case studies and workshops are being piloted in engineering, business, and public policy classrooms. We have also been working with the Center for Advancing Teaching and Learning through Research at Northeastern and our external assessment advisor, Dr. Michael Loui, to develop assessment instruments and evaluation schemes that can be used across all of the cases. We now have a scheme that covers the common ethical concepts introduced in the cases—distributive justice, weighting/balancing risks, moral status, the precautionary principle, responsibility to report, and exploitation. The evaluation scheme is based on the framework presented by the Ethical Reasoning Value rubric published by the Association of American Colleges and Universities and will be applied to five separate classes of students over the coming year in order to test learning outcomes. This project grew out of the team’s experience with trying to fit existing engineering ethics cases into a life cycle–based framework. To provide a baseline for evaluating the new case studies, a review of learning assessments was carried out in spring 2015 for a mechanical/industrial engineering course, which currently uses a case study–based ethics module about the Bhopal chemical disaster, and retrospectively for the 150+ students who have passed through the course over the past several years. Review of assignments and responses informed the creation of case study teaching notes and the draft evaluation scheme. Continuing assessment will allow the project team to adjust the cases and teaching materials as necessary and add further instructional guidance where learning objectives are not being met.

Case studies are one tool that can be used to address the context and impact of engineering ethics, and have been proven to be an effective teaching and learning method.

These case studies cover a variety of engineering disciplines, professional situations, and personal dilemmas and focuses on several areas of moral pedagogy. They were developed for use in higher education, but may also be of use in other settings.

To accommodate many educational levels, the case studies are divided between Beginner, Intermediate, and Advanced cases. They are written in parts so that educators have the flexibility to use them as is best suited to their teaching content and environment, and all cases permit and encourage the integration of technical content. Along with learning and teaching notes, the cases contain suggested questions and activities as well as supplementary materials.

They are aligned to the Engineering Council and Royal Academy of Engineering’s Joint Statement of Ethical Principles and the expectations of the 4th Edition of the Engineering Council’s Accreditation of Higher Education Programmes and are therefore appropriate for UK teaching and learning contexts. They are, however, easily adapted for use in other countries.

Guidance articles are also available to help situate the case studies in an educational context and to signpost to additional research and resources on engineering ethics.

In developing the cases and articles in this resource, the authors and advisory group took into account recent scholarship on best practices in teaching engineering ethics through case studies. They also reviewed existing case study libraries in order to add to the growing body of material available on engineering ethics.

This web site is being developed to give undergraduate Mechanical Engineering students real engineering examples and experimental data in their lecture courses. This is done by using design calculations or experimental data completed by ME seniors in their design courses, lab courses, honors thesis, and Co-op (with permission from the company). Results from graduate student research or published experimental data are also used. This allows students to become familiar with data analysis and the inherent uncertainties in experimental measurements before entering the laboratory courses.

The case studies are formatted as a homework assignment. In some cases, the entire case study would be assigned for homework. In other cases, there are several similar sets of data and only one set of data would be assigned for homework. Each case study includes a description of the objectives, the experimental facility, and the experimental procedure used to obtain the data. When possible, photos and video clips are used to show the facility and experimental procedure. From the web site, students can download engineering drawings of the models used in the experiment and spreadsheets containing the experimental data. The web site describes the data analysis required in the assignment. Included in the data analysis may be nondimensionalization, statistical analysis, plotting the experimental data, and comparison of the experimental results with correlations found in the text.

Two cases about Hertz claimed top spots in 2021's Top 40 Most Popular Case Studies

Two cases on the uses of debt and equity at Hertz claimed top spots in the CRDT’s (Case Research and Development Team) 2021 top 40 review of cases.

Hertz (A) took the top spot. The case details the financial structure of the rental car company through the end of 2019. Hertz (B), which ranked third in CRDT’s list, describes the company’s struggles during the early part of the COVID pandemic and its eventual need to enter Chapter 11 bankruptcy. 

The success of the Hertz cases was unprecedented for the top 40 list. Usually, cases take a number of years to gain popularity, but the Hertz cases claimed top spots in their first year of release. Hertz (A) also became the first ‘cooked’ case to top the annual review, as all of the other winners had been web-based ‘raw’ cases.

Besides introducing students to the complicated financing required to maintain an enormous fleet of cars, the Hertz cases also expanded the diversity of case protagonists. Kathyrn Marinello was the CEO of Hertz during this period and the CFO, Jamere Jackson is black.

Sandwiched between the two Hertz cases, Coffee 2016, a perennial best seller, finished second. “Glory, Glory, Man United!” a case about an English football team’s IPO made a surprise move to number four.  Cases on search fund boards, the future of malls,  Norway’s Sovereign Wealth fund, Prodigy Finance, the Mayo Clinic, and Cadbury rounded out the top ten.

Other year-end data for 2021 showed:

  • Online “raw” case usage remained steady as compared to 2020 with over 35K users from 170 countries and all 50 U.S. states interacting with 196 cases.
  • Fifty four percent of raw case users came from outside the U.S..
  • The Yale School of Management (SOM) case study directory pages received over 160K page views from 177 countries with approximately a third originating in India followed by the U.S. and the Philippines.
  • Twenty-six of the cases in the list are raw cases.
  • A third of the cases feature a woman protagonist.
  • Orders for Yale SOM case studies increased by almost 50% compared to 2020.
  • The top 40 cases were supervised by 19 different Yale SOM faculty members, several supervising multiple cases.

CRDT compiled the Top 40 list by combining data from its case store, Google Analytics, and other measures of interest and adoption.

Artificial Intelligence (AI) has been advancing rapidly, and its capabilities can be leveraged for effective and optimal utilization of resources in the chemical process industries (CPI), especially the oil and gas industry. These industries need to conduct several process safety management (PSM) studies as part of their risk management strategy, which requires significant resources, including competent personnel, time, and money.

This article explores how different PSM studies can benefit from AI. The successful implementation of AI depends on the availability of quality labeled data and plant-specific training of the AI model. In the CPI today, most plant data is not available in a labeled format and cannot be used directly in machine learning algorithms. Therefore, after carefully reviewing the data availability and resource requirements for PSM studies, this article suggests starting with rule-based AI implementation as an initial step, followed later by the application of machine learning and deep learning concepts.

Implementing AI in PSM studies can be a game-changer, and this article discusses the necessity and limitations of such an undertaking. This article also proposes a roadmap to help the CPI integrate AI into its PSM studies with ease and efficiency.

What is hindering the adoption of 3D concrete printing?

The first three-dimensional printing using concrete came in 1995, and the technology has evolved over the past three decades. Just the cost savings alone should make 3D concrete printing attractive as new building technology. From reduced waste materials to faster construction times, it seems to be an efficient solution when combined with the freedom of design elements. While there has been buzz around some pioneering projects, overall there has been resistance in the industry, with some skepticism about the technology and a general lack of understanding by the public. Author Haidar Alhaidary encourages 3DCP adoption in his paper for the Journal of Architectural Engineering , “Three-Dimensional Concrete Printing as a Construction Automation Strategy and Assessments from a Case Study Building.”

Alhaidary begins by outlining current challenges in the construction industry, starting with declining productivity, the skilled labor shortage, ineffective project management, and unsustainable industry; and concluding with what he calls the rise in construction demand (the dichotomy between wanting to limit building to save the planet and addressing demands for urban expansion). He continues his argument by acknowledging the well-known resistance that the construction industry has shown toward change, and its hesitancy to adopt new, sometimes inferior, technologies. He uses the Middle East Engineering Technologies villa, completed in 2020, as a case study and addresses several challenges encountered during construction, as well as community criticism. Numerous techniques and technologies exist to automate construction-related tasks, which include the latest three-dimensional concrete printing (3DCP) methodology. These automation techniques seek to solve the entrenched problems in construction, such as declining productivity, shortage of skilled workers, ineffectiveness in project management, unsustainability, and the increase in construction demand. The 3DCP has further benefits, such as quicker construction times, design freedom, and more predictability. However, there is resistance to adopting 3DCP in the industry and by the market. With experience from a 3DCP organization and a case study on a 3DCP villa, this paper presents the major challenges that hinder this technology’s adoption and use, important lessons learned from the villa construction, and highlights of the local community’s critical feedback.

How For-Profit Hospitals Billed More Aggressively Than Nonprofits For Medicare Outlier Payments: A Case Study From the Early 2000s

American hospitals account for the largest nonprofit sector in the United States, but while most U.S. hospitals are nonprofit, a significant portion are for-profit. Policymakers and researchers long have questioned whether nonprofit hospitals behave differently than for-profit hospitals. In a recently released National Bureau of Economic Research working paper that examines this question, we made use of a now-resolved loophole in Medicare payment policy to understand how nonprofit and for-profit hospitals exploited this loophole, and what they did with the resulting financial windfall.

What was the loophole? Medicare used hospitals’ list prices (also called “charges”) to determine how much hospitals should get in “outlier payments” — which reimbursed hospitals beyond Medicare’s usual fixed-price contract for providing especially resource-intensive care. Because the Centers for Medicare and Medicaid Services could not observe the true costs of this treatment, they relied on those list prices, giving hospitals the opportunity to inflate them.

We used a novel approach to find hospitals that gamed the system with inflated charges for outlier payments during this time period and identified 180 such hospitals. The gain for these hospitals was substantial, resulting in a windfall of $3 billion in Medicare payments and $12 billion when including all insurers’ payments over this time period. (For perspective, consider that the total spending on the Women, Infants and Children’s (WIC) program in 2022 was $6 billion.)

Both for-profit and nonprofit hospitals took advantage of the loophole. But we found that the type of ownership was highly predictive of gaming, with the for-profit hospitals more likely to exploit the loophole than the nonprofit hospitals. The for-profit hospitals also were more likely to pocket the money rather than reinvesting it in patient care.

Hospitals That Gamed the System Derived Direct and Indirect Revenue Benefits

We found that weaknesses in government contracts can produce ripple or “spillover” effects that may far exceed in magnitude the direct revenue increases, based on our comparison of the revenue trends of the “gamer” hospitals versus those of a matched set of hospitals that did not inflate list prices for outlier payments. As our analysis showed, the impact of the loophole extended beyond the increased payments from Medicare to include increased payments from private insurance too.

This spillover — adding up to $12 billion — perhaps resulted from private insurers using similar payment designs and negotiating contracts based on list prices. The higher spending by private insurers, which then likely was passed on to employers in the form of higher insurance premiums, greater cost sharing for employees, and even salary decreases, highlights the importance of considering spillover effects when determining whether investments in oversight or enforcement actions are justified.    

Nonprofit Hospitals More Likely to Use Extra Money on Operating Costs

It is unclear exactly why the for-profit hospitals were more likely to take advantage of the Medicare outlier payment loophole. We hypothesize that managers of for-profit hospitals had more incentive to maximize revenue since they could distribute profits to themselves. We also hypothesize that managers who joined nonprofits versus for-profits differed in their financial motivation.      

What is clear, though, is that the nonprofit and for-profit gamers spent the extra revenue in different ways, consistent with predictions from organizational theory. Nonprofits directed about 75% of the gaming revenue to operating costs, with no increase in compensation for senior executives. We detected a modest but statistically significant improvement in the mortality rate at these nonprofit hospitals.

In contrast, for-profit hospitals transferred all of the excess revenue off their balance sheets, with no effect on patient care. For the largest and most important gamer, the Tenet Corporation, we showed that the excess revenue dramatically increased compensation for its highest-paid executives at the peak of gaming. Compensation more than doubled, from $6 million to $12 million and stock options ballooned to $92.5 million.

The system also engaged in stock buybacks, which resulted in nearly a billion dollars transferred to shareholders. In the case of for-profit hospital systems, this evidence strongly suggests that investing in more stringent oversight is likely to carry high social value.

Loopholes Will be Exploited and For-Profit Hospitals May Warrant Greater Scrutiny

Although the findings look back to a loophole closed two decades ago, they are particularly salient now. According to the US GAO, Medicaid and Medicare made more than $100 billion in improper payments in 2023.

The issue of improper payments has taken on additional policy significance in the aftermath of large payouts to hospitals for care related to the COVID-19 pandemic. Congress authorized nearly $200 billion in pandemic-related emergency assistance for healthcare providers.

There are concerns that some hospitals and other entities abused this and other COVID-era relief programs. The appropriate policy responses — in response to such “gaming” — depend on how the marginal dollar of excess revenue ends up being allocated.

For example, if hospitals direct excess revenue to patient care, policymakers may have less to fear from loopholes. However, if the excess revenue has limited benefits for patients and workers, it would support devoting greater resources to contract design and payment oversight.

The working paper, “Turbocharging Profits? Contract Gaming and Revenue Allocation in Healthcare,” was published June 2024 by the National Bureau of Economic Research and authored by Atul Gupta , Ambar La Forgia, and Adam Sacarny.
