Modern equipment at home has reduced the labour work and increased dependency on this equipment.  People don’t have enough time to exercise or even get enough sunlight. Nowadays people are living very unhealthy lifestyles.  

Unhealthy living conditions have increased the contraction of people to various diseases like obesity, diabetes, heart attacks, hypertension, etc. This has alarming implications in the near future. So it is very important to focus on our health as much as we focus on our work. Moderation in food habits, daily exercise, and balanced work-life can surely make a big difference to our health and body. When a person stays mentally and physically fit, his actions and decisions are more practical and logical and hence he is more successful in life. Furthermore, good health has a direct impact on our personality.

It's crucial to consider how much self-control you have to keep a healthy lifestyle. Research reveals that changing one's behaviour and daily patterns are quite tough. According to the data, whether a person has a habit of smoking, drinking alcohol, doing drugs, or any other substance, it is extremely difficult to quit. A study found that 80% of smokers who tried to quit failed, with only 46% succeeding.

Every year on April 7th, World Health Day is celebrated. The World Health Organization (WHO) hosted the inaugural World Health Day on April 7, 1950, to draw the entire world’s attention to global health. 

Every year, the World Health Organization (WHO) comes up with a new theme for public awareness, such as "Support Nurses and Midwives" in 2020. This supports the situation of COVID-19, where healthcare workers are saving lives day and night without worrying about their health.

They are constantly using these technological items, oblivious to the fact that they may harm their health. Teenagers are frequently discovered engrossed in their electronic devices, resulting in mishaps. 

The usage of electronic devices frequently results in anxiety and hostility. Excessive usage of these products has been linked to cancer, vision loss, weight gain, and insomnia. 

Emotional development is another crucial component that should not be disregarded because it determines whether or not a person is healthy. An emotionally healthy person should have a solid sense of logic, realisation, and a realistic outlook.

Many people spend years dreaming about their retirement. Unfortunately, today’s workers will likely work longer, suffer greater economic uncertainty, and might have poorer health status compared with retirees in previous generations. Preserving good health during the working years is associated with a more consistent employment record, greater financial resources, and reduced risk of disease. Making smart financial decisions as a younger adult also translates to improved finances in retirement. While many people are aware of these relationships, many continue to make poor health choices. Employers and lifestyle medicine professionals can both work to improve financial well-being in retirement. Employers can offer effective worksite financial wellness programs and promote participation in retirement savings programs. Physicians and other health providers can foster healthy behaviors, encourage preventive services compliance, and help adults foster overall financial and health well-being. Adopting a healthy lifestyle as early as possible would increase the likelihood that today’s workers will enjoy financial security in retirement.

Introduction: Changes in Retirement

Advances in medicine, technology, and well-being have resulted in greater longevity in the United States and therefore the need to plan for a retirement that could be many years longer than seen in previous generations. 1 According to SoFi.com , the average American retires at age 64. 2 Since today’s average life expectancy is 79 years, 3 most people will spend about 15 years in retirement. While Social Security data show that most Americans claim retiree benefits as soon as they are eligible, 4 current workers in the Baby Boom generation expect to work longer than earlier cohorts of workers. 5

Unfortunately, about one-half of retirees have no retirement savings, and the median savings for near-retirees (age 55-64 years) is only about $12 000. As of 2017, half of private sector employees had access to employer-based retirement benefits—either defined benefit pension-style plans or defined contribution 401(k) types of plans—which has been at a consistent rate since 1979. 6 However, only 52% of households with a head of household or spouse age 25 to 64 years old participate in an employer-based retirement plan when one is available to them. 7 Ethnic and racial wealth disparities were exacerbated in the Great Recession of 2007 to 2009, 8 and the COVID-19 pandemic has resulted in the highest level of unemployment in the United States since data collection began in 1948. 9

The prevalence of having multiple (2 or more) chronic conditions from the 2018 National Health Interview Survey for US adults with private health insurance coverage was 15.7% for 18- to 64-year-olds, while the prevalence for those individuals age 65 and older was 63.2%. The chronic diseases included in that survey were arthritis, cancer, chronic obstructive disease, coronary heart disease, asthma, diabetes, hepatitis, hypertension, stroke, and weak or failing kidneys. 10

At this time, approximately 10 000 baby boomers turn age 65 each day, with that trend expected to continue for many years. 11 Whether or not a person has enough savings is a key factor in the decision to retire. 12 Another major factor is whether or not a person is healthy enough to continue working if they do not have enough savings to retire. To shed light on that topic, the purpose of this review is to identify the relationship between health and finances in employed adults nearing retirement age.

Employee Health and Wealth

Employers have long supported worksite wellness programs to promote employee health. Goals typically include managing health care costs, disability, and worker productivity. Although there are altruistic motives for instituting these programs, a chief goal has often been health care cost control. 13 These wellness programs have resulted in healthier employees who may or may not live longer, but who likely will have more productive years of life. 14 - 17 Numerous studies have established the link between health risks measured by a health risk appraisal and economic outcome measurements such as health care costs, pharmacy costs, and productivity costs. 18 - 22 Similarly, research has documented the links between multiple health indicators and employee income over time. 23

Employees who participate in employer-based wellness programs are likely to be healthier in their retirement years and therefore will need additional retirement savings to continue their active lifestyle. In contrast, employees who have medical conditions or health risk factors placing them at increased risk of disease in retirement will likely need additional retirement benefits to cover their medical expenditures. A Kaiser Foundation study found that in 2012 an estimated 1 in 3 Americans had difficulty paying their medical bills within the past year. They were paying previous bills over time or were unable to pay their medical bills at all. 24 A majority of these people who had trouble paying medical bills had health insurance but had out-of-pocket expenses for health care not covered by their insurance or cost-sharing expenses for insurance deductibles and co-payments. For all people, financial resources are critical to the quality of the retirement years.

A review of the literature found very little published research on the association between health and retirement saving contributions. Gubler and Pierce reported on 401(k) contributions by 414 employees working in 8 multistate laundry locations. They found that the decision to contribute to a 401(k) retirement plan predicted whether an employee who had a basic health risk appraisal and 42 blood screening tests acted to correct their poor physical health risks in the following year. 25 In that study, participants received a financial incentive of an approximately 15% decrease in their health insurance premiums depending on their base salary ($1.75 to $11 per month) to complete a health risk appraisal. Furthermore, they found that employees who contributed to their 401(k) were 27% more likely to show improvements in their health risks and biometric abnormalities than noncontributors in the following year. A total of 208 out of 213 (98%) of these employees with 2 screenings in 2010 and 2012 had at least one abnormality and 53/213 (25%) had at least one “severely abnormal” finding. The only intervention for the abnormalities was referring the employee to their doctor for follow-up, which points to the importance of a strong ongoing relationship between aging workers and their primary care physician.

Brown et al found that households approaching retirement in recent years held significantly more debt than households nearing retirement in the previous decades. 26 This result was also found by Lusardi et al, who analyzed data from the long-term Health and Retirement Study as well as the National Financial Capability Study. They concluded that people approaching retirement now hold more debt and suffered more financial insecurity than past generations. 27 A 2016 study found that the health of Americans aged 50 to 70 had not improved over the past 2 decades, 28 with other studies showing that the health of adults in that age category may have actually worsened during that time period. 29 , 30

Changes in Social Security over the past 50 years have increased the “normal” retirement age. 31 , 32 Therefore, upcoming retirees will retire and claim benefits at a later age than previous cohorts, despite the fact that their health is worse according to several measures. The consequences of this combination of factors may lead to other outcomes such as an increase in people applying for Social Security disability benefits, or an increase in health care costs for employers whose employees are working until later ages. 33

It is known that postretirement medical expenditures are related to health. A Medicare population was asked to respond to the following question: “In general, compared to other people your age, would you say your health is: excellent, very good, good, fair or poor?” Responses were the following: 18% as excellent, 26% as very good, 30% as good, 17% as fair, and 7% as poor. Medicare expenditures had a marked inverse relationship to self-assessed health ratings. In the year after assessment, age- and gender-adjusted annual expenditures varied 5-fold, from $8743 for beneficiaries rating their health as poor to $1656 for beneficiaries rating their health as excellent. Hospitalization rates followed the same pattern: respondents who rated their health as poor had 675 hospitalizations per 1000 beneficiaries per year compared with 135 per 1000 for those rating their health as excellent. 34

Health and wealth are both important resources for living a happy and successful life. People in poor health often die young and spend thousands of dollars for medical care that could have been invested for retirement. On the other hand, those who practice recommended health behaviors are more likely to exceed average life expectancy and need a larger nest egg to ensure that they do not outlive their assets. Health and personal finances are both closely associated with happiness. Research suggests that 4 factors strongly predict happiness and well-being in most cultures: health, financial stability, work, and other productive pursuits, and family and friend relationships. 35 - 37 This concept was reiterated in a 2017 study of retirees and the changing nature of retirement with many individuals choosing or having to work after the traditional retirement age. The most influential resources related to retirement outcomes in that study were financial resources, health, and, in some cases, social networks. 38 Studies indicate that people are happier when they are healthy, employed, or doing something productive; have strong social relationships; and are financially secure. Costly health incidents can deplete a household’s savings, leaving them vulnerable. 39 Happiness has been associated with longevity. 40 For example, very happy people have a 6% lower risk of death over a given follow-up time period than people who are pretty happy, and 14% lower risk of death than those who are not happy.

There is abundant literature reporting that a reduction in health risk factors is associated with less short-term disability, absenteeism, improvement in on-the-job productivity (“presenteeism”), and lower health care cost expenditures. 41 - 43 Of equal importance is whether changing health risk factors in older adults results in changes in these metrics. The percentage of people in the United States who work and people who want to work has increased markedly in both the 65 and older and 75 and older age groups according to the American Association of Retired Persons Public Policy Institute. 44 The workforce participation rate for 65 to 74 was 17.5% in 1996 compared with 26.8% in 2016. For 75 and older, the rate jumped from 4.7% in 1996 to 8.4% in 2016. 45 An April 2013 Gallup survey had similar findings. A total of 1005 nonretired, working people were asked when they reached retirement age “will you continue working and work full-time; continue working part-time; or stop working altogether?” 46

  • 15% said they would work full-time, and 40% of those said it was because they wanted to, not because they must.
  • 61% said they would work part-time; almost 55% of those said they would do it because they wanted to.
  • 22% said they would retire and stop working altogether.

The current average retirement age for men and women is 64 and 62, respectively, while current workers report they anticipate retiring at age 67. 47

What Can Employers Do?

The need for health care benefits is one of the factors that workers between the ages of 50 and 65 have cited most frequently when asked to identify major reasons to work in retirement. 48 This suggests that access to employer-sponsored health insurance benefits may be used by organizations to retain skilled and experienced workers. To address this employee need, some employers may offer health insurance to employees who work part-time. Employers that can provide these options to employees with increased longevity will be addressing an essential need of the skilled worker in the coming years.

We believe that incorporating health metrics into retirement planning earlier in life might have a favorable impact on employee retirement saving contributions. 49 In economics, the concept of time preference is the current relative value placed on some item today compared to receiving it at a later date. That is the trade-off that employees need to consider when choosing to save for retirement as well as make healthy choices today. Researchers attempt to capture this difference in today versus later value as “time discounting” because individuals choose to forego future wealth or health in favor of immediate consumption or suboptimal choices. 50 There are benefits to employers as well. According to Towers Watson’s 2012-2013 Global Workforce Study, “Employers that successfully invest in supporting both employee health and employee confidence in their financial well-being drive a surprising high return on investment for all stakeholders.” 51

Some employers may choose to implement flexible or phased retirement policies, which allow aging workers to transition to part-time work or shared jobs as they approach retirement. In a simulation study, a model was created in which workers knew about their future health condition (based on personal, family history, and community risks) when they were young, and adjusted their savings accordingly to improve their future financial situation. The simulation allowed for flexible or phased retirement that helped minimize the need for welfare programs for the future retirees. 52 A 2017 analysis of the Health and Retirement Study data found that more than half of older Americans were utilizing some combination of 3 strategies for their retirement including changing employers later in life, reentering the labor force after an initial retirement, or reducing their hours in career employment after traditional retirement age. 53 The authors noted that this flexibility is beneficial to many workers but that it also reflects the hardship endured by some retirees in poor health or with insufficient financial resources.

Employers who contribute to the health care costs of their retirees may also consider offering a health promotion program for that population. In 1992, Bank of America randomized 5686 of its retirees into a full health promotion program, questionnaire-only, or control groups to determine if the program had any impact on health habits, risks, or utilization. 54 After 1 year of follow-up health risk scores and health care costs decreased significantly in the program group compared with the other 2 groups. There are also altruistic reasons for providing a wellness program to employees who retire, which may attract motivated workers to companies. Similarly, General Motors also offered a wellness program to its retirees in 1996 in an effort to improve health status and contain health care costs in that population. 55

The Health and Retirement Survey data show that financial literacy is common among retired individuals, who often rely on professional financial planners, retirement seminars, and financial calculators. However, results show that younger adults are not as well informed. 56 It is not clear why so many people fail to plan for retirement at a young age but some of the suggested reasons include low salaries, and burdensome student loan debt, no access to an employer-sponsored retirement savings program, parenthood, and homeownership financial strain. Employers can improve the situation by sponsoring workplace seminars on retirement planning and encouraging participation in 401(k) plans with matches or other strategies. Incorporating health metrics into retirement planning sessions earlier in life may have a favorable impact on employee retirement savings contributions as well. 57

What Can Lifestyle Medicine Practitioners Do?

A study in the United Kingdom attempted to identify common health problems that forced older workers into retirement due to poor health. The study found that older workers who reported depressive symptoms or impaired physical mobility were at increased risk of making an early transition out of work. 58 Lifestyle medicine practitioners targeting these conditions may enable older workers to remain in the workforce until they have reached full retirement age. Encouraging healthy lifestyle behaviors is another way lifestyle medicine physicians can improve older adults’ lives. A UK study of older adults found that physical activity tends to decrease around age 55, which has a detrimental impact on health and well-being in retirement. 59 Activity with a social component may be the key to keeping that age group active.

Encouraging positive behaviors after retirement is another important way for lifestyle medicine to impact this population. A large population-based cohort study in Australia found that retirement was associated with positive lifestyle changes such as quitting smoking, increases in physical activity, reductions in time spent sitting, and improved sleep. 60 A large corporation using a population-based study in the United States offered wellness programs to all active and retired employees in one area of the country. Participation rates were monitored anonymously over 4 years. It was found that retirees who participated in the wellness program activities during their employment also had higher participation rates as retirees. 61 Health professionals may wish to capitalize on this time of major transition to focus on healthy choices and behaviors.

Conclusions

Retirement is something that most people spend a significant amount of time thinking about. Unfortunately, the reality of retirement may not match each person’s dreams. The research summarized in this article reveals that today’s workers will have to work longer, have more economic uncertainty, and have poorer health measures compared with retirees of previous generations. Maintaining good health during the adult years is associated with a more stable employment history, more financial reserves, and reduced risk of disease. Making wise financial decisions as a younger adult also translates to improved finances in the retirement years. Even though most people are aware of these relationships, many people still choose to make poor health choices such as smoking, living a sedentary lifestyle, and eating a poor diet, as well as less than ideal financial choices such as incurring large amounts of debt, outspending their income, and not saving for retirement. Employers and lifestyle medicine professionals can both contribute to improvements in this situation. Employers can encourage participation in worksite wellness programs and participation in 401(k) savings programs. Physicians and other health providers can foster healthy behaviors, preventive service compliance, and community support for individuals. Implementing those changes as early as possible would increase the likelihood of adults being able to enjoy a long and healthy retirement.

There is abundant proof that money does buy better healthcare, better health, and a longer life. On the other hand, good health is linked to the ability to create wealth, as well. Thus, there are strong links between several health indicators over the average life span of individuals and their available income.

Scientists have already summed up the results of multiple studies to show that wealthier people live longer, have less chronic disease, and maintain a higher level of function well into their old age. Reduced death rates, a longer life expectancy, and a lower risk of lifestyle conditions modified by income, such as obesity, smoking, hypertension, and asthma, are all concomitant benefits of having a high disposable income.

This is explainable in terms of the better living conditions available for the wealthy, coupled with better preventive and therapeutic healthcare, which means they do not become as seriously ill or remain so for as long as those who are poorer. Disability or chronic ill health resulting from such conditions may dog a person throughout life.

The effects of poor healthcare due to poverty and social inequalities may extend across generations to affect the unborn and children. For instance, a lack of vaccinations may leave the unborn prone to teratogenic infections such as rubella and measles. Malnutrition during pregnancy may cause low birth weight and developmental defects.

Similarly, maternal deaths may be higher for a lack of access to affordable quality healthcare. Increased spending on healthcare means less money for other expenses, reduced savings, and thus a state of unpreparedness for health and other emergencies. Poor health also reduces productivity, reduces earning capacity, impacts learning, and reduces emotional well-being.

Among high-income countries, American men and women live for an average of 76 and 81 years, respectively. Since they have the highest rates of injury and disease globally, their old age is medically and financially demanding. Interestingly, despite high incomes, Americans have high rates of homicides, vehicle accidents, sexually transmitted diseases, obesity and diabetes, alcohol- and drug-related deaths, lung disease, cardiovascular disease, and disabling autoimmune and joint diseases.

Inequalities Breed Ill-health

Social inequalities form a powerful weapon in keeping the poor in that condition, even while helping the rich get richer. It is noticeable that the disparity between the rich and the poor has become glaringly apparent in the USA. Poor households with negative or no wealth make up over a fifth of all households, according to 2016 data, which leaves them without resources in case of unemployment, an unexpected illness, or other expenses.

With healthcare being costly, many Americans lack insurance. Even so, lack of healthcare is responsible for one in ten deaths before the expected lifespan. The problems mentioned above cause the remaining premature deaths among poor Americans. Poorer adults are likely to have hypertension, obesity, infectious conditions, heart disease, and psychiatric illness.

Poverty is intimately linked to discrimination and systemic racism, that account for much of the difficulty experienced by the not-wealthy. For instance, poor or minority neighborhoods receive less investment and have lower property prices. The residents find it almost impossible to access bank loans and are at much higher risk of being unfairly suspected or treated by the police and judicial system.

Health inequalities are related to differences in the rates of various conditions like asthma, diabetes, obesity, and cancer, as well as of societal ills like violence, drug abuse, and alcoholism; to differences in the healthcare available to different groups and ethnicities; and differences in the healthcare insurance available. As a result, the poorest US counties have a life expectancy as much as three decades lower than those in the richest – a striking illustration that shows how near the poor in America are to the unhealthiest populations in the world.

Inequalities Breed Stress

Social inequalities, seen most sharply in the US among all developed countries, appear to be an independent factor that reduces life expectancy, overall health, and well-being, despite a higher income. Studies show that “ people in more egalitarian societies live longer, experience less violence, have lower rates of obesity and teen pregnancy, are less likely to use illicit drugs and enjoy better mental health than their counterparts in countries with a wide divide between rich and poor .”

Stress is a powerful weapon and is equally experienced by the rich and poor. The effects of social inequality, seen most sharply in the US, are mediated via chronic stress, which exerts a deleterious effect on the heart, blood vessels, immune responses, and brain.

In addition to the physical aspects of poverty, which can be seen, even the rich suffer from chronic performance anxiety, demands to keep up appearances and increase their wealth through fierce and unending competition, and loss of trust. Inevitably, such a society breaks down due to the impassable barriers of suspicion and mistrust between the people who make up the community.

Concerning children’s health, less unequal societies are notable for higher rates of childhood immunization, accidental deaths among children, lower rates of addiction to tobacco and alcohol, and higher rates of educational achievement.

The Way Out

According to social scientists, policies to reduce inequality are health- and wealth-producing policies. Entrenched commercial interests that market foods and products that foster ill-health must be fought and removed from decision-making areas at the top. This is unlikely to happen anywhere in the world, however, which means that individuals and communities must take their health and wealth into their own hands.

Thus, multipronged interventions will be necessary. Small Steps to Health and Wealth is one stepped program that provides life coaching for poor and/or sick people to help them change their habits to fit their long-term goals.

People should also take charge of their health as far as possible, with ongoing maintenance, regular scheduled check-ups, and building good habits. The same goes for wealth creation, especially since a longer lifespan implies the need to build enough capital to last one’s life.

Public attitudes to health and wealth are underpinned by public knowledge of what drives these entities. In turn, public attitudes drive public behavior. Behavior is one area that psychologists can help change, and their skills should be harnessed to drive healthy and productive behavior.

The situation that favors the creation and perpetuation of economic disparities can be corrected only by advancing wealth to invest among those who have been disadvantaged by all these factors, allowing them to move forward by providing opportunities in education, banking, the police, and justice systems, housing and including their input while framing policies.

Microloans for small businesses, help to rent better houses in better neighborhoods, or to buy their own homes, helping adults gain insight into financial management, coaching to help achieve specific goals and assistance with getting a minimum wage adequate to live a decent lifestyle while simultaneously providing assistance to climb the ladder where appropriate via on-the-job training.

Focusing on children is a highly effective strategy since it ensures a good start for each individual and has the most long-term ramifications of all. This includes universal quality prenatal care, early education with all kids entitled to attend good preschools, and improving the educational system to achieve high academic and learning skills. The classrooms are also the fertile ground where educators can instill healthy attitudes and behaviors that promote diligence and creativity.

Along with equalizing healthcare access, the quality of care given to the poor needs to improve. The most important thing is to target opportunities to promote health since this occurs earlier in the course of events rather than focusing on outcomes alone.

Better treatment for the sick would keep caregivers at work for a longer time, thus increasing overall revenues. Proper distribution of economic growth by investing in public services would benefit the poor and take the strain on safety-net services.

The balance between health and wealth has become a significant issue in the community, particularly in a social setting of health and wealth. As we sail through different challenges in contemporary life, it is evident that our reliance on good health and financial stability significantly plays a pivotal role in influencing the quality of life one needs (Anvari-Clark & Rose, 2023). As a healthcare giver, I have encountered several health and wealth gap cases, particularly on patients with significant financial challenges in accessing essential medical services. The health wealth gap is an inescapable issue that establishes real-life situations affecting people’s access to essential healthcare services and physical health. My perception of health and wealth issues is that everyone should have equal access to healthcare services regardless of financial status (Bhugra, 2018). Thus, my perception is mainly on the professional encounter and personal ethics that recognize equity and social fairness. Thus, this paper intends to discuss social health and wealth and how they relate to each other in the critical aspects of human life.

In my workplace, when I came across a patient named Mark, I understood that the patient was facing financial constraints. Hence, despite her severe condition, Mark was denied access to healthcare services due to a lack of finances to facilitate his treatment. I realized that Mark had no insurance coverage due to his financial constraints; therefore, he could not afford one, which would have taken care of her condition. Hence, the lack of insurance and lack of finances prompted delayed medical interventions, therefore deteriorating his condition (Anvari-Clark & Rose, 2023). Mark’s incident made me realize the difference between health and wealth; sometimes, lack of financial resources can lead to severe fatalities, especially for individuals with low income in society. Since there is no timely intervention from doctors and nurses, receiving appropriate healthcare services is problematic (Bhugra, 2018). Hence, situations like this call for policy changes to ensure that lack of finance would help determine the quality of an individual’s healthcare.

I am obligated to contribute with a different perspective on the health-wealth gap in the principles of equity and social justice. Health is an important factor in any human being; therefore, it qualifies as an essential human right that should not be denied to anyone, regardless of their financial status (Sapolsky, 2018). Since the physical health of one person in a community displays the health of the whole community. Hence, if that person is denied access to medical services and, for instance, transmits the disease to another person, it will turn out to be disastrous, and as a result, a pandemic will be more problematic to control. Thus, fair distribution of medical services is critical in ensuring general public health.

The impact of the social economic aspect on an individual’s physical and psychological well-being is very significant in the course reading, according to the study conducted by Ratcliffe and McKernan (2012). Mckernan argues that the social background from which the child is brought up can develop a weak foundation that will have a detrimental effect on the child’s future academics, behavior, and physical well-being. Furthermore, the social factors of health include education, employment, and housing, which are closely associated with financial status. People with higher incomes generally have better educational backgrounds and better jobs that, in one way or another, have a positive impact on their physical health (Ratcliffe & McKernan, 2012). Lifestyle aspects also play a pivotal role since wealthy individuals can afford recreational activities, gym memberships, and healthy foods. At the same time, those with lower incomes will generally face difficulties sustaining a healthy lifestyle due to limited financial resources as they cannot afford most of the important things to nurture their health condition. Therefore, it is of great essence to address the health wealth issue so as not to confine it to only wealthy people who are already staying fit with the assistance of their finances.

Accessing healthcare services is essential, but it remains a significant issue even in countries with advanced healthcare facilities and systems. The issue of health and wealth cuts across the globe remains a significant issue since even where there are advanced systems, medical expenses for individuals with low income remain a challenge that usually leads to delayed or no medical services at all, leading to casualties at the expense of money (Chokshi, 2018). Hence, addressing the issue of the health wealth gap will require a wide range of strategic interventions intended to minimize the rate of economic disparities, enhance healthcare accessibility, and provide social services by resolving aspects of affordable education, equal job opportunities, affordable housing, and financial support.

Lack of capital can lead one to underlying stressors that can cause mental problems. The continuous worry of where to find resources for food, clothes, and other essential basic needs can have an adverse effect through the development of stress and, therefore, lead to mental health issues. This worsens the situation since mental health services have limited access to people with low or no income (Chokshi, 2018). Furthermore, low-income people live in congested areas that are often polluted, lack basic amenities such as sound drainage systems and housing, and lack green spaces, rendering them more vulnerable to various physical health conditions due to exposure to an environment that is not conducive to human settlement.

It is essential to understand that the health wealth issue is not only limited to specific regions but also a worldwide issue that is being established in different ways in different communities. Therefore, addressing this issue requires a general approach considering global and local aspects (Bhugra, 2018). If not resolved, these issues can become a menace and bring about lasting effects that will trigger poverty, enhance problems for future generations, and confine economic mobility among the people.

In conclusion, the health-wealth gap remains a reality deeply rooted in our societies with a significant influence on the lives of different kinds of people depending on their financial status. From my real-life experiences in the healthcare provider sector, I have witnessed how economic inequalities influence an individual’s health outcomes. My perception of the health wealth gap issue is based on the engagement of different stakeholders that are involved in offering healthcare services to ensure equity and fairness that is impacted by the personal and professional ethics and moral standards in addressing health, which is fundamental in the life of a human being with dignity and decorum. By appreciating the existence of the health wealth gap, I am in support of the changes in policy and strategies that will ensure this issue is addressed once and for all and create a society where health is not only for the rich but also for all people irrespective of the person’s financial situation.

Health vs Wealth: The Impact of Mining on Pregnancy Outcomes

In 2011, the Supreme Court of India implemented restrictions that ultimately banned mining for over 150 iron ore mines due to environmental concerns. We study the impact of these mining restrictions on pregnancy outcomes, finding reductions in stillbirths and low birth weight deliveries for births in regions subject to the restrictions relative to control regions. We do not find statistically significant effects on medically terminated pregnancies or maternal anemia. As mechanisms, we show that the restrictions on mining reduced overall household consumption but improved air quality in treated districts. We also find evidence of income-selected fertility, with mothers in treated districts showing higher income quintiles despite overall declines in household consumption. Our results suggest that the environmental benefits of the mining restrictions outweighed the local economic costs in terms of pregnancy outcomes. These findings have important implications for understanding the health-wealth trade-offs in resource extraction and policies related to India's energy transition and future mine closures.

The continuum of care for maternal health in Africa: A systematic review and meta-analysis

Background The continuum of care for maternal health (COCM) is a critical strategy for addressing preventable causes of maternal and perinatal mortality. Despite notable progress in reducing maternal and infant deaths globally, the problem persists, particularly in low-resource settings. Additionally, significant disparities in the provision of continuous care exist both between continents and within countries on the same continent. This study aimed to assess the pooled prevalence of completion across the maternity care continuum in Africa and investigate the associated factors. Methods Relevant articles were accessed through the EMBASE, CINAHL, Cochrane Library, PubMed, HINARI, and Google Scholar databases. Funnel plots and Egger's test were employed to assess publication bias, while the I-squared test was used to evaluate study heterogeneity. The inclusion criteria were limited to observational studies conducted exclusively in Africa. The quality of these studies was assessed using the JBI checklist. Data extraction from the included studies was performed using Microsoft Excel and then analysed using Stata 16 software. Results A total of 23 studies involving 74,880 mothers met the inclusion criteria. The overall prevalence of women who successfully completed the COCM was 20.9% [95% CI: 16.9–25.0]. Our analysis revealed several factors associated with this outcome, including urban residency [OR: 2.3; 95% CI: 1.6–3.2], the highest wealth index level [OR: 2.1; 95% CI: 1.4–3.0], primiparous status [OR: 1.3; 95% CI: 2.2–5.1], planned pregnancy [OR: 3.0; 95% CI: 2.3–3.7], and exposure to mass media [OR: 2.7; 95% CI: 1.9–3.8]. Conclusion The study revealed that only 20.9% of women fully completed the COCM. It also identified several factors associated with completion of the COCM, such as residing in urban areas, possessing a higher wealth index, being a first-time mother, experiencing a planned pregnancy, and having access to mass media. Based on the study's findings, it is recommended that targeted interventions be implemented in rural areas, financial assistance be provided to women with lower wealth index levels, educational campaigns be conducted through mass media, early antenatal care be promoted, and family planning services be strengthened.
